Covid -19 Information, Policies and Health intake form.
I would like to thank you for contacting me. You should know I take your health and safety very seriously. I have instituted Covid policies for the safety of both you and me.
These include the following:
Massage by appointment only.
If you have been exposed to covid in the last 14 days please do not come in. If you are not feeling well or have any covid symptons please do not come in. I am happy to reschedule.
One person at a time in my studio. No one can wait with you.
I clean my room and supplies before and after each session.
You will be required to fill out and sign my Covid 19 Declaration form as well as my usual health form.
At this time I am not seeing anyone who has had the virus with any symptoms.
Masks are required as you walk into the building and during your session until you are back in the parking lot.
Your temperature will be taken when you enter using a non-contact thermometer.
